Senior Care Options

If you are a senior and have a Boston Medical Center* provider you may qualify for the Senior Care Options (SCO) program.

With SCO you can receive care that combines health services with social support.

As a SCO provider, Home Care with Grace, will coordinate and deliver acute, institutional, community-based long-term care, mental health, and substance abuse services.

Each of our members is assigned a personal care manager from Home Care with Grace who is solely dedicated to coordinating your care and services. To make sure your unique needs are met, a care plan will be developed by a team of your doctors, nurses, social workers and other healthcare professionals; as needed. Your care manager will help to schedule medical appointments, make sure you have transportation and any interpreters you may need.

Similar to our Adult Foster Care program, these services are provided by a Home Care with Grace staff member instead of a live-in caregiver.
*Home Care with Grace currently contracts with Boston Medical Center (BMC HealthNet SCO Plan)
